32. A 62-year-old man is brought to the emergency department because of a 2-hour history of left anterior chest pain that began after he finished raking leaves. Initially, he thought that he had strained a chest muscle. Rest did not relieve the pain, and it began to radiate to his back. He has a 20-year history of hypertension treated with β-adrenergic blocking agents; he is not always compliant with his medication regimen.
