This form is signed by you in front of an official of your medical school, consulate or notary public and then to be sent straight to ECFMG from his office, which I believe is not possible in many cases but they have asked for it.

this is also not possible in my case. but what i can do is that sign this form in front of notry public officer and send it to my college official with all completed forms like 344 or 345 so that my college can send it to ECFMG office.

if this is signed in front of notary public officer, then should it also be certified by the official of medical school?

No you can't do that. You have to get is signed in front of the officials I told you about. They verify you signature by matching it with your passport or any other photo id by govt. I want to mention that ECFMG will not send form 186 to your college. It will send form 345 only with the copy of your diploma and transcript. It will tell you when it will send so you can facilitate the process in your college and then the college has to send it back to ECFMG.

i just called ECFMG and asked about this form. they said that i can sign it in front of notary public officer and send it to them.
