residency: postgraduate training in a specialty... about 3 years...eg Internal medicine fellowship: training after you conclude your specialty, a subspecialty... about 3 more years... eg Cardiology

There is no postgraduate degree for residency. You're MD, and e.g. pediatrician, internist, psychiatrist, etc.

Better ask for observership and with letter of acceptance and letter from CS go to the consulate... nothing is for sure but is always better to show what your real intentions are... people at the consulate have a weird screening for giving visas.

hi ethics, I am just surprised you dont have someone from ur college already in US. If you dont mind where ru from? about ur question, its better to apply now as observerships are very difficult to get it back. then u can ask for visa only for CS. I dont think u can officially do an observership on visitors visa.
