I`m taking my exam on Feb 17th (hopefully!! have no intention to postpone it further).

Doing pharma right now. Have the rest of the night to go thru last 3 sections (6 chapters).

1) Patients with pheochromocytoma in OR ~ which inhaled anesthetic cannot be used? Halothene ~ sensetizes heart to catecholamines

2) Inhaled anesthetic causing megaloblastic anaemia in chronic exposure ( staff working in poorly ventilated dental OR) ~ Nitrous Oxide

3) Inhaled anesthetic with low blood gas partition coeff, but patients hold their breath due to pungent smell ~ not used for induction of anaesthesia ~ Desflurane

anti-seizure drug causing peripheral neuropathy ~ requires F/U examination for deep tendon reflex activity in lower extremities (in chronic use)
~ Phenytoin

Epileptic patient on treatment enrolls for methadone maintainance program, withdrawal effects more pronounced ~ due to enzyme inducing effects of phenytoin/carbamazepine --> increased metabolism of methadone

Although Ethosuxamide has a t1/2 of 40 hours the dosage is twice daily to reduce the severity of GI distress

Withdrawal is least likely to be a problem with which anti-seizure drug?
~ Ethosuxamide (absence seizure Tx withdrawal is easier than withdrawal from drugs used for tonic-clonic or partial seizures)

Carbamazepine not used in absence or myocloinic seizures ~ reported to worsen them

Effective blood level for Ethosuxamide and Valproic acid is 50-100 mg/L ; seizure recurrence if plasma level drops below 50 mg/L in chronic Tx (caution: theraputic window narrow, so plasma level monitoring required)

Q. 3. What other enzyme (other than Alcohol DH) contributes to ethanol metabolism and @ what blood level of ethanol???

Ans.1. Due to limited supply of NAD+ required in ethanol metabolism ---> fixed
capacity of ethanol metabolism = 7-10 gm/h

Ans.2. Facilitates action of GABA @ GABA(A) receptor
Inhibits binding of Glutamate to NMDA receptor

Ans.3. MEOS (Microsomal Ethanol Oxidizing System) @ >100 mg/dL of blood level
of Ethanol

Activators:
-BZ
-Barbis
-Zolpidem, Zaleplon

-Alcohol
-Alphaxolone(a steroid aneasthetic)
-Etomidate
-Propofol
-Halothene
-Anticonvulsants (Gabapentin, Vigabartin)
-Ivermectin !!!

Blockers:
-Picrotoxin (dirct channel blocker)
-Bicucullin (inhibits GABA binding to receptor)

Was planning to nail pharma, which I`ve been trying to for past few days .... couldn`t make it sad ... sad ... too freaking bad sad ... will allow myself one more day (today/13th) only, plus mycology and parasitology from micro .... and some virology if possible .....

I was supposed to start B/S today ... but still stuck with Immuno sad sad .... will finish immunology and immunopatho from goljan tonight ... and possibly the rest of the pharm

To be honest, I`m in no place to recommend, you know!! I`d suggest you to follow the path of 99'ers smiling face .... but if you ask MY opinion, I`d say I found HY histology more informative than Kaplan, at least go thru the chapters for epithelial tissue/cell junctions, Connective tissue, bone, cartilage and whatever you think is missing in Kaplan. It kinda combines pathology so I thought it was more helpful for correlation.

But if you are doing goljan big book, you might find it redundant smiling face

I`m starting to lose tempo sad .... and I`m having hard time with Physician/patient relationship sad .... sometimes the answers don`t make any sense to me sad .... anyways, will start doing micro today. Today and tomorrow is for micro ....

I guess I`ll need one more day for micro... I have three classes this week .. and two assignments sad ... hate it sad ....

Anyways, will do general and systemic bacteriology today with antibiotics (from pharm) and bacterial genetics nod

I never finished a few chapters from pharm and B/S, but have to squeeze them in somewhere in my schedule rolling eyes .... anyways, whatever it takes, will nail micro by tonight nod

Well, I haven`t started doing questions yetsad .... except for the end of the chapter questions from kaplan+/- Lange book series. For some of the subjects, this is my first read... like immuno, B/S and part of pharm and most of the patho ... which is pretty crazy ... but I need to juggle my work, school and USMLE prep and I need to be done with it before my midterms sad sad

Planning to finish all the subjects by Jan 30th and then a marathon UW (7 blocks a day) ( 7 hour for solving+ 7-10 hours explanations) ....

will definitely update how I`m surviving this :P

Thanks mytime .... I know I shud be doing questions, but to me, it doesn`t make sense to solve q`s before knowing the stuff thoroughly .... I know i`m taking a huge risk, but I started doing blocks from Qbank and my cumulative was quite disappointing disapproval so, changed the plan .... but I`m planning to do two blocks from qbank everyday after I finish U/W ... too far-fetched I know ... but let`s see ....


I wish I started my prep a few mnths earlier ... could be a part of a great teamnod ... good going you guys smiling face ... love the spirit...

yup mad ... and guess what , I `m still wavering b/w going thru them all over again and moving on to some other subject ... wish I didn have to go to class today shaking head ...

will finish the remainder of pharma and B/S today after I do microbial genetics and cases ....

It`s been such a bad day ... tried to finish metabolism but still left with heme and nucleoprotein and lipid metabolism ... spent three good hours after some mindless assignment ... I just envy the people who get a 4/5 month block absolutely free of work and worries and whatever .... i know i cant afford to be a weakling ... just hope i`m not underestimating the boards ... good luck and goodnight everyone...
