A 52 yo nulliparous woman comes to the OP clinic with a left palpable lump whitch is hard but not tender . Menarche was at age 12 . Menopause was 3 years ago . Her last mamogram was 1 year . Her mother had breast cancer diagnosed at age 70 . On examination , a 3 com movable non tender mass is palpated in the retroaureolar region of the breast . No nipple discharge . The skin above the lesion appear normal . What is the next test in diagnostic ? a ) mammography b ) FNA c ) excisional biopsy d ) ultrasound e ) radical mastectomy f ) radiotherapy and chemiotherapy

we will start with having a mamography and then a core, incisional biopsy...if cancer found....we should do a radical mastectomy because it is retroareolar..nodes will be biopsied and if they are positive we should do systemic chemotherapy and hormonal therapy if nodes are receptor positive. Radiations are usually not done if the whole breast is removed. Plz let us know the answer.

Well guys , I put this question just to see what would you pick because I am confused about that . Kaplan said that you find a palpable mass in a woman over 40 , you shoud do a mammography first and than FNA ( as you all understand ) but Goljan 2 said that mammography is to pick up non palpable mass and if a mass is palpable in a woman aver 40 , the nest step in management is to STICK IT ( FNA ) If someone could clear that for me , I would appreciate that Thanks in advance !

in kaplan lecture proff has explained that mammography is to be done even in presence of palpable mass to examine other part and req tissue can be taken from that part also
