If the pt says he has wife and family is it appropriate to ask about no of partners? How do I ask it?

Detail sexual history - ( i need to ask a few personal Q about ur sexual health. all that you tell me will remain between us )
Are you sexually active ?- if yes with my wife - ask have you ever had multiple partners ?
have you ever had a sexually transmitted disease ?
If yes - have you been tested for HIV in past ?
