Hi, I was going thru the Goljan questions recently..one answer mentions that the adenocarcinoma has surpassed the frequency of squamous cell in esophagus and lung. I am a very old graduate~ 10 years..at that time it was squamous cell. Can u please let me know whats the correct one? Thanks.

Adenocarcinoma of the esophagus is now considered to be 50% or a little more , due to the increase in barrets esophagus. The other half or less is squamous cell of the esophagus. For the lung adenocarcinoma is now the number 1 cancer. This is because the advent of the filter. While adenocarcinoma is not specifically linked to smoking, the filters blocked out large carcinogens that would initially wedge themselves in the mainstream bronchi. Thus sqaumous cell was commonly a hilus/ mainstream bronchus type of cancer and most common for a long time. These filters eliminate the large carcinogens and allow the much smaller ones to pass through. These smaller ones can reach all the way to the periphery of the lung where they deposit. Thus there has been an increase in adenocarcinoma which is the peripheral cancer. periphral location, pucker the pleura .etc This is the explanation goljan gives and is correct for the current time.
