A 5-year-old African-American boy is brought to the office by his mother for a painful right knee, fever and chills over the last 2 days. The child has a history of pharyngitis 2 weeks ago treated with a 10-day course of penicillin G. On examination, temperature: 38.8C(102F); PR: 102/min; the right knee joint is swollen and erythematous with a decreased range of motion. Laboratory findings are significant for leukocytosis and an elevated ESR. X-ray examination of both the right hip and knee joints is normal. What is the most appropriate treatment?


A. Bed rest for 7 days with right knee joint in position of comfort
B. Repeat a ten-day course of penicillin G
C. Bed rest for 7 days & a ten-day course of penicillin G
D. Bed rest for 7 days & aspirin
E. Right knee arthrocentesis & IV nafcillin treatment

i think this one is septic joint..but i am having confusion regarding the diffrence between history and sign n symptoms of septic joit vs osteomyelitis....i know may sound really stupid...

i agree with sarika,i will go with arthrocentesis and IV nafcillin treatment.

can't confirm but exclude either it is rheumatic fever.
aspirin is OK for protection of the joint.

what is the answer, robin?
