A 30-year-old African-American male comes to your office with complaints of swelling and pain in his right knee since the last 15 days. He says that he first experienced pain when he was playing football 15 days back and twisted his leg. He felt something popping in the knee at that time but ignored it. However, over this period the pain and swelling has been increasing and he feels sudden pain on extending his leg. On examination, the right knee is swollen and tender along the medial side. Full extension of the right knee is not possible due to sudden pain during terminal extension. Snapping can be felt in the right knee on tibial torsion with the knee flexed at 90 degrees. X-ray of the knee joint is normal. Which of the following is the most likely diagnosis in this patient? A. Anterior cruciate ligament injury. B. Posterior cruciate ligament injury. C. Medial meniscus tear. D. Medial collateral ligament tear. E. Lateral collateral ligament tear.
