A 3-week-old African American boy is brought to the Emergency Department because of a generalized seizure 2 hours ago. The infant is highly irritable with incessant high pitched crying. The infant's weight is 2.5 kg (250 gm below birth weight), blood pressure is 70 /40 mm Hg, pulse is 145/min and respirations are 50/min. Laboratory results show:
Blood glucose 120 mg/dL
Urea nitrogen 50 mg/dL
Serum sodium 170 mEq/L
Serum calcium 8.5 mg/dL
Serum magnesium 1.5 mg/dL
Which of the following is the most likely cause of this infants seizure
a) Hypocalcemia
b)Hypoglycemia
c)Hypomagnesemia
d)Intracranial hemorrhage
e)Meningitis

resp 50 per min,high pitch cry,irritable,low bp,sz all pointing towards septicemia/meningitis and the Q is about wat exactly caused the pt sz...hmmmmm
i`ll go with Meningitis...
if thew options had hypernatrimia i`d have certainly opted for it....
hypocal will cause tetany which is diff fron sz .. option a
ther is no hypoglycemia... ....b
hypomanesemia will cause arrythmias.....c (it might have been accompanied with hypokalemia which is not give here, and accompanied decrease or absent bowel sounds)
intracranial h`rrage is not the possibility here...d
any other explanation is welcome, thx

It's meningitis... he is septic hence the low BP... his hypernatremia could be due to severe dehydration and Na retention to increase the BP...

how can an intracranial h`rrg cause tachypnea of 50 per min, i`ll stick to meningitis
