a 36 yrold gravida 2 para 3 comes to the physician for pre natal check up. she recently moved to the area and states she is 7 months gestation. according to an us performed at 16 weeks , she is at 30 weeks. does not use tobbacco alcohol or drugs.Examination shows a fundal height of 25cm.temp 98.7, b.p of 145/90 mm hg, pulse 75/min and respirations re 16/min. fetal heart tones re heard by doppler. repeat us shows a bipareital diameter consistent with dates, and abdominal circumference below the 10th percentile. which of the following is the most likely cause of these findings??

a.chromosomal abnormalities
b. intrauterine infection
c.hypertension
d. gross fetal anomalies
e.inaccurate dates

please could u explain this too

b. intrauterine infection can cause asymmetrical IUGR...BP consistent with dates but abdo circum below 10th percentile shows just that.

C - Hypertension
Definately assymetric IUGR(Head Sparring)... head receives all the nutrients from the blood... however most infections that can affect the fetus does it early in gestation and lead to symmetric IUGR... I believe its Maternal hypertension - which will affect the fetus late in gestation and lead to a nutritional deficit.

I think the diagnoses here is OLIGOHYDRAMNIOS , not iugr b`coz fetal age is consistent with the gestational age, so we must seek the condition associated with oligohy... and i found 2 dds here
1st intrautrine inf - but there is no history s/o such findings
2nd gross fetal anomalies like renal agenesis, and telipus equino varus post partum option D , i think i`ll go with D
any other explanation is welcome, GL
