20. An 80-year-old man is brought to the emergency department on an 80-degree day 2 hours after a syncopal episode while playing golf. He plays golf twice weekly. An ECG obtained at his last examination 6 months ago showed occasional premature ventricular contractions (PVCs). On arrival, his temperature is 38°C (100.4°F), blood pressure is 150/90 mm Hg, pulse is 80/min and regular, and respirations are 22/min. Cardiac examination shows a grade 3/6, late-peaking systolic ejection murmur. No bruits are heard over the carotid arteries, but the upstrokes are delayed. Which of the following is the most likely cause of the syncope? A ) Aortic stenosis B ) Heat stroke C ) Hypertensive cardiomyopathy D ) Platelet emboli E ) PVCs

cant be heat stroke as temp is less than 105.delayed carotid and the murmur suggest aortic stenosis.no carotid bruit so not emboli.PVC are usually benign.HTN can affect heart but aortic stenosis is most likely here
