For each patient described below, choose the SINGLE most likely diagnosis from the above list of options. Each option may be used once, more than once, or not at all. 1. A 37-year-old cyclist is knocked from his bicycle by a car. He is brought to the Accident and Emergency Department and after initial assessment and intervention he is noted to have left upper quadrant tenderness. 2. A 25-year-old woman car driver is involved in a front impact accident. She develops central, epigastric and shoulder tip pain three hours later and is noted to have linear transverse bruising from the lap belt above the umbilicus. 3. A 47-year-old scaffolder falls about four meters onto a scaffold pole injuring his right side. After initial assessment he is found to have diffuse right-sided tenderness. A ten-minute intravenous urogram (IVU) shows no excretion on he right side. 4. A 19-year-old is stabbed with a 10 cm blade in the left upper abdomen. After remaining stable for 20 minutes his blood pressure drops to 90/60mmHg. An ultrasound scan shows no free abdominal blood. Options A.Deudonal Rupture B. Aortic rupture C. Bladder rupture D. Bowel rupture E. Cardiac tamponade F. Diaphragmatic rupture G. liver rupture H. Mesenteric vascular injury I. Pancreatic contusion J. Renal contusion K. Renal pedicle avulsion L. Splenic rupture M. Urethral rupture

Scary questions 1 - splenic rupture 2 - ???? 3 - renal pedicle avulsion 4 - could be splenic rupture if the splenic capsule is intact... but why couldn't it be cardiac tamponade - 10cm knife could go far... maybe I am overthinking... lol

L spleenic rupture F shoulder tip pain favouring diapragmatic rupture although duodenal rupture is more common K renal pedicle evulsion E bc upper abdomen extends upto nipple so possibility of cardiac temponade is there favoured by no free fluid in abdomen.
