A 35-year-old HIV positive African American male comes to the physician because of deterioration of vision over the past week. He says that he initially experienced eye pain and mild conjunctivitis, which was followed by rapid progressive visual loss. Examination of his eye reveals marked keratitis, and funduscopy shows widespread, pale, peripheral retinal lesions and central necrosis of the retina. Which of the following is the most likely causative organism for this patient’s condition?

B. Cytomegalovirus
C. Herpes simplex

its Herpes simplex, cytomegalovirus will not cause conjunctivitis and keratitis.

CMV can cause keratitis especially HIV pts. Pls refer these links:

Only Herpes and VZ almost always produces a prodrome of conjuctivitis and keratitis before causing 'NECRTOTISING' lesions in the retina---->'PAINFUL' loss of vision...
CMV is unique coz it is seen at CD4 < 50 in HIV pts and very rarely we see a prior conjuctivitis and keratitis before a full blown picture on the retina.. moreover CMV retinits appear as yellow,granular,fluffy opacified lesions,prefering areas close to retinal vessels -->retinal hemmorhages seen on fundoscopy--->'PAINLESS' loss of vision

So I go for C--------->Herpes simplex
