A 60 yr old man who had been diagnosed 3 days before as having had an acute MI suddenly developed mitral insufficency. This was most likely due to a) coincidental rheumatic mitral stenosis b) extension of infarct to mitral valve leaflet c) rupture of an infacted interventricular septum d) rupture of an infarcted papillary muscle e) subacute bacterial endocarditis on mitral valve

no the answer is d) rupture of an infarcted papillary muscle: this allows a mitral cusp to fly free and produce mitral insufficency
