Is anyone filling the state license forms? I have a few quesries. if some one who is filing the application for state licesing or has info can reply..
Q1 - I have a form given to me by the program which is different to the one available on State licensing website.. what should I do?
Q2- If I complete and send my application for processing now - along with the ECFMG CVS...will it be accepted even though my medical school will take some time to do their part of the verification/
Q3 - the CV that is required,, is it along any specific format or should I make a new CV with jsut my details and experience? how many pages are okay.
Q4 - on the section where it says: " Do you hold licesnse to practice medicine or surgery in another jurisdiction" - does it mean US license or can I write the Pakistan license and also UK licesnse i.e GMC registration
Q5 - Where can I find individual states requirements... i mean the no. of weeks during clerkships that is required by individual states. ( I have 2 weeks of Psychiatry in medical school and 2 weeks in Internship) the only verification of my Psych rotation is by my medical school, the internship certificate just says Medicine Internship.
I will appreciate if someone replies.

1. Check with the program.
2. Most likely - states are used to the fact that the school stuff will come separately
3. Check with the state for what they want
4. US licensure
5. State's web page for information.

i just called the PC of my program and also the State board...
The state board said that it will be a long process and applications are sent back many times to applicants for changes or corrections.. so I should send the form and they will email me if something more is required or needs correction... hope all goes well in the mean time
