can we work as a physician assistant on a voluntary basis on a visitor visa to US

You may not work in any capacity, even as a volunteer, if you enter the US on a Visitor's Visa. Some hospitals may let you do it, but generally, you will not be allowed to do so. But some places are quite lax so maybe you'll luck out.
