should i write down like
PMH then write the information
and allergies then the information
then social hx......and so on, or can i just write all the information mixed all togther with the present illness. this saves allot of time.or i'm supposed to address evey topic i need to give the history about? please reply

It is best to do things in an organised fashion. Follow a check list so that you don't miss anything:
Presenting Complaint
History of Presenting complaint
Description of problem
Past Med History
Allergies to meds
Past Surg History
Past Fam Hx
Past Social Hx
Review of Systems
Physical exam
- Relevant exam
- Cardio exam
- Listen to chest
Do it systematically to make sure that you don't miss anything. This also makes it easier and more efficent for you to write up the patient note. Be sure to include the sub-headings mentioned above in the patient notes.
