A homeless 38-year-old man has a 3-month history of decreased appetite, generalized wasting, a low-grade fever, and worsening cough productive of bloody, mucopurulent sputum. He has a 3-year history of alcohol abuse. X-ray films of the chest show consolidation and cavitary lesions in the apices of the lungs. Which of the following is the most likely inflammatory and/or immune response in the lung? A ) Endothelial cell proliferation, vascular budding, fibroblasts B ) Epithelioid cells, multinucleated giant cells, caseation necrosis C ) Fibroblasts, collagen deposition, replacement of intrinsic tissue D ) Macrophages, lymphocytes, hypergammaglobulinemia E ) Neutrophils, vascular congestion, exudate 39. A 30-year-old woman with type 1 diabetes mellitus has increasing vulvar pruritus and a vaginal discharge. A wet mount preparation of the discharge is shown. Which of the following is the most likely causal organism? A ) Candida albicans B ) Chlamydia trachomatis C ) Herpes simplex virus D ) Human papillomavirus E ) Trichomonasvaginalis 19. Poliovirus can be grown in cultured monkey kidney cells through repeated passages, resulting in virus that is no longer neurovirulent. This virus can be used as a vaccine because it has become A ) attenuated B ) defective C ) inactivated D ) latent E ) recombinant 39. A 5-year-old boy is brought to the physician's office because of pharyngitis. Culture of a swab from the throat grows a β-hemolytic gram-positive coccus in chains. What to do , if you find the correct answers please post them , you really made me curious and above all intrigued. Q17- definetely DiGeorge sdr --> definitely narrowed mediastinal shadow and decreased peripheral lymphocytes , gamma globulins I think are normal ( they were made by the liver) so the answer should be C. Q37 - definetely Tuberculosis answer B Q39 - could be either Candida(A) or Trichomonas(E) , I`d go with A though Q19 - it is Cystitis , the most common org is E.colli so my answer would be D Q29 - I have no ideea , bu I`d pick A Q39 - definetely D Q41 - no ideea could be paracoccidioidomycosis , toxoplasma , cryptococcus ,taenia solium/saginata , paragonium weatermani , I really can`t put my finger on one of these (and other) ; so guessing I would go for E ( implies Taenia solium - prevalent in Latin America --> cyticercosis ) Answer E

only thing I can add is that 29 is a for sure. Attenuation means that the virus is still alive and able to cause a immunogenic response but lost its virulence. This is how they make the sabin or oral polio vaccine. Therefore agreeing with brutus

T solium is pork tape worm. When considering a tape worm disease you want to know who the definative host is and who is the intermediate. Normally the intermediate host (pig) eats the egg, in the pig the eggs develop into larvae. The definitive host is the human and we eat the pork meat (undercooked) that contains the larvae. in this case you would develop into a normal tape worm in the intestinal tract. In this case however, the human has become the intermediate host, in this case we ingest the eggs and larvae develop and encyst with in us. Common presentation is siezures with cyst like lesions (cortical calcification in the brain. I just thought I would shed light on this since there were no new questions posted. Hope that clarifiesd the last one. Another case where humans are intermediate host is hydad cyst disease(echinococcus granulosus). Generally the disease will be much worse when there we are intermediate host rather then the definative.

. Inactivated virus Like killed bacterial vaccines, these vaccines contain whole virus particles that have been treated (again, often with formaldehyde) so that they cannot infect the host's cells but still retain some unaltered epitopes. The Salk vaccine for polio (IPV) is an example. 6. Attenuated virus In these vaccines, the virus can still infect but has been so weakened that it is no longer dangerous. The measles, mumps, and rubella ("German measles") vaccines are examples. The Sabin oral polio vaccine (OPV) is another example. It has advantages over the Salk vaccine in that
