a 63 year old female presents to clinic with compliants of palpitations. She says that for the past 3 weeks she has noticed that her heart has been pounding and then the heart beat suddenly stops. This is more frequent at night. she denies taking any heart medications but does taking insulin for her diabetes. examination reveals an alert female with no distress. EKG show "atrial flutter". BP 145/75. The next step in her medical management is
a. administer lidocaine
b. cardioversion
c. carotid massage
d. administer verapamil
e. administer quinidine

The answer is d. Can any one tell me why it's not c:carotid massage?

THIS PATIENT HAS ATRIAL FLUTTER AND HYPERTENSION, SO VERAPAMIL TREATS BOTH

CAROTID MAY DECREASE BLOOD PRESSURE AND HEART RATE BUT I HAVE NOT EVER HEARD IT MAY STOP ATRIAL FLUTTER.

management of atrial flutter are rate control and anticoagulation

USMLE123, where is the q from?

In kaplan, the initial should be carotid massage, in Harrison's, its cardioversion.

CCB is right, but not the initial step.

I don't know why D.

cardioversion only when patient is unstable and when atrial flutter is newer in onset

webjee ,you have a point,the treatement for atrial flutter would be carotid massage according to the algorithm for treatement of tachycardias by A.H.A. but you cant keep her on carotid massage and if it will revert to sinus rythm AF will recur,AF is not easy to control.
Another point in the Q. it says MANAGEMENT AND THE POLICY in the non emergency setting is to go for the long term management.
LAST POINT FOR THE VALUE OF CAROTID MASSAGE WITH A.F. IT WILL UNCOVER THE SAWTOOTH WAVES ON THE EKG IF IT WAS NOT APPARENT SO IT HELP BUT NOT HERE.
