i come across some questions that need some clarification.. appreciate for anyone who can shed some light on them.. thanks

1. can u put orhtostatic sign on the w/u? if not, how do u check on that assuming we do not need to measure BP twice during SP encouter?

2. is it wise enough to ask 'any other symptoms' during encounter? would the SP give away other symptoms.. if they just said 'no', can we assume that there r absolutely no other symptoms.

3. Do we need to do Dix-Hallpike manuever during encouter? can we just write that on w/u?

4. how important is the order of w/u or d/d? suppose u put CBC, U/A, U/S-ABD on w/u but out of order, how would they grade that?

Hi transcripto, I´m preparing for cs on jan..., to your qs:

1.Yes you can order orthostactic vitals on the w/u, just let the SP you will do it later, so that he marks his thing. Actually taking them takes a lot of time and you will miss a lot of other important stuff.

2.You can ask about associated symptoms in relation to the chief complaint, maybe the SP will give you a few, but you HAVE to ask for specific ones. As for ROS I think it´s better to be specific if the symptoms could be related to your DDs, if the chance of relation is low I would just ask something like "any urinary problems instead of any hematuria, foamy urine, dribbiling, etc.

3.I would do Dix-Hallpike since it´s almost diagnostic and it´s really fast to do it. Check the imagesadhttp://www.cmaj.ca/cgi/content/full/169/7/681)

4.You should write the w/u in order of importance and of simplicity, begining with most specific test, least invasive and least expensive and the DD in order of probability. I´m not sure how would the cs people would grade your note if I doesn´t do it that way, maybe the will not check or mark a box in reference to "order in DD or w/u".

Transcripto I don´t know when is ur test, but I have gone through the cases againg and it seems there is no time to do Dix-Hallpike on the PE and it´s better to order it in the w/u
