22 yr homosexual male had severe colds accompanied by copious bloody sputum with greenish flecks, fever, overall loss of energy and skin rashes. He underwent a physical examination which included a full blood workup. His overall cell count was within normal range with exception of his CD4+ cells which was 150/mm3 I) The heavy sputum may have been due to an opportunist infection caused by a) Chlamydia pneumonia b) Strep pneumonia c) Mycoplasma pneumonia d) Mycobacterium tuberculosis e) Influenza A virus II) The pt's profile best fits that of an individual infected with a) HIV b) EBV c) Influenza A virus d) coronavirus e) adenovirus III) Drugs used in treatment of the pathogen include all except: a) AZT b) ddI c) ddC d) interferon e) acyclovir

I) d II) a III) e Infection by HIV and AIDS are not easily diagnosed because the incubation period may vary from months to years. Most diagnostic procedures for the virus detect circulating antibodies which emerge slowly in some cases. Opportunistic infections, such as M tuberculosis occur at higher rate in IC individuals. Deep, productive coughing with blood are indications of possible mycobacterial infection and therfore warrants further confirmatory tests. Surrogate markers for AIDS take into account several factors because CD4+ T cell counts fluctuates among individuals. A count of less than 300cells/mm3 is considered a sign of immunosuppression. 2-microglobulin, a component of major histocompatibility complex; neopterin, produced by white cells; and interferon are all elevated in HIV pts. Delayed type hypersensitivity is suppressed. Fever may be result of pathology due to opp infections and therefor not a reliable surrogate marker for HIV AZT, ddI, ddC and interferon [i am not sure bout this, the explanation listed this but it better be omitted from choices, sorry bout confusion  ] have all been used for tx. Acyclovir is not effective against HIV
