A 72-year-old woman has had no major illnesses throughout her life. She has had several syncopal episodes over the past two weeks. Over the past two days she has developed shortness of breath and a cough with production of frothy white sputum. On physical examination she is afebrile. Her blood pressure is 135/90 mm Hg. She has no peripheral edema. A chest radiograph reveals a more prominent left heart border most consistent with left ventricular enlargement, but the other chambers do not appear to be prominent. There is marked pulmonary edema. Laboratory studies show a total serum cholesterol of 189 mg/dL. Which of the following is the most likely diagnosis?

A Acute rheumatic fever

B Mitral valve stenosis

C Atherosclerotic aortic aneurysm

D Calcific aortic stenosis

E Infective endocarditis

I guess is D - the symptoms and Rx sounds like aortic stenosis and MCC in elderly is calcification.

why not C. syncope could be TIAs from atherosclerosis . high blood press could also be linked to atherosclerosis. aortic insufficency could have caused the left heaert failure. On the other hand , she couldnt maintain a higher bloor pressure with aortic stenosis. i'm not sure.

Her B.P is 135/90mmHG she is likely to have more of a non-working valve other anything else cos this is the only period where is having Signs of syncopy. Moreover her Cholesterol is 189mg/dl which is not relative with Atherosclerotic Disease
