A young undergraduate college student, majoring inbotany, went on a field trip with her classmates to search the Delaware shoreline for representative plants. The area included both shoreline and a deciduous forest inhabited by small rodents. the students entered both areas. A week following the trip she experienced flu like symptoms which included high fever, headaches, muscle aches, general malaise and stiff neck. A large 3-4 cm circular red blotch developed on her lower leg and became sensitive to touch and gradually increased in size

I) On basis of history, sx closely fit a preliminary dx of:
a) Weil's disease
b) Leptospirosis
c) Lyme disease
d) Epidemic relapsing fever
e) Plague

II) The disease is transmitted by:
a) louse

III)Causative agent of disease is
a) Borrelia burgdorferi
b) Leptospira interrogans
c) Leptospira icterohemmarrhagiae
d) Borrelia recurrentis
e) Rickettsia rickettsii

IV) The untreated disease will usually resolve itself, however several months following initial infection, a small percentage of infected individuals wil experience a 'second stage' of infection involving
a) visual damage
b) renal failure
c) neurologic abnormalities
d) disseminated skin lesions
e) synovial infection and arthritis

V) A third stage of disease, occuring months to years after intial infection, may occur involving
