A 17 YR OLD BLACK FEMALE HAS HAD IRREGULAR PERIODS SINCE MENARCHE AT AGE 13 .HER PERIODS OCCUR AT SIX TO SEVEN WEEK INTERVALS.LAST FROM SEVEN TO TEN DAYS AND R ACCOMPANIED BY SEVERE PAIN FOR THE FIRST THREE DAYS .PHYSICAL EXAMINATION IS NORMAL.NEXT STEP IN MANAGEMENT IS?

What about sickle cell crises? Can that manifest as painful irregular menses? In that case will not CBC be the rigth answer? She is a black woman....sickle cell disease....dunnoo....

wel i guess for menorrhagia we give combined ocps , prog only pills lead to breakthro` withdrawl bleeding, no h/o contact nor amenorrhoea so no preg test ,
i think the ans shd be CBC , for she has flow for 7-10 days she may be having anemia

I agree. CBC should be the next step in management.
The girl is bleedng her hear out (Menorrhagia) and no other complaint.

but CBC is nt going to help the symptoms .. more over the physical examination is normal n i guess tht include the examination 4 pallor

I go with ultrasound
what about endometriosis? or other bleeding pathology
because it does not have the choice to give a trial of hormone ultrasound would be better.
in CBC you will see maybe anemia, but you need to correct the problem first, also with cbc you will not see if is VWD or sickle cell
temparature sounds good but no
pregnancy well she has it since 13
