this is an nbme qs I found on this forum can u please answer it and expalin it why u choose the option,I will be grateful

blood pressure is 95/60 mm Hg, and pulse is 165/min
and regular. The
lungs are clear to auscultation. Which of the

A) Accelerated idioventricular rhythm

i am confused between junctional tachycardia and paroxysmal SVT!
history suggests two thngs, one these paroxysms are not associated with any AV blocks, so V-tach and idioventricular rythms are out! and last time it was controlled with an IV med, probably adenosine! there are other differentials in which paroxysm will be controlled with adenosine but they won't be too regular!
but i think i will go with paroxysmal SVT!
need more input so will check it out!

The Patients Pulse is 165/min,within the rage of Paroxysmal supraventricular tachycardia (130-220). WHile in Accelerated junctional rhythm , The junctional rhythm initiates within the AV nodal tissue. Accelerated junctional rhythm is a result of enhanced automaticity of the AVN that supersedes the sinus node rate. During this rhythm, the AVN is firing faster than the sinus node, resulting in a regular narrow complex rhythm. These rhythms may demonstrate retrograde P waves on ECG findings, and the rates can vary from 40-60 beats per minute.
