Marked difference between PAO2 and PaO2 before and after administration of O2 points that there is any one of them - Shunt
- V/O2 mismatch
- Diffusion defect
PDA and patent formen ovale cause shunt when there is reversal Eisenmenger complex ,not initially . So I think it is Diffusion defect. Best of luck

Its E .. Same as Leo said .. Diffusion Defect .. !! And dont know why there are two Pa02 given .. And .. as the data tells that after infusion of 02 .. the PA02 stats improve .. indicatiing Diffusino impairment .. due to structural physiology .. !!
