i have not done a clerkship so i have no idea. by the way are u still in medical school or did u graduate. cause if you have graduated you cant do clerkships. then you can do only externships/observrship. will it help?? no one can say. residency application is a dynamic thing you cant say one thing will help you and other wont. Dont stress too much on scores. great scores are big plus but are not everything. . but people who get better scores will mostly do better. even though u may not get it in university program you can try community programs

There are 2 kinds of IMGs: Those with citizenship/GC who can apply to progs that do not sponsor visas, which is a pretty good thing coz you rule out all the IMG population that require visa (maybe more than 10000 applicants). If you belong to this category, then you could have good chances with your 78, especially if you get a good step 2 score. If you belong to that group that require visa, J1/H1, then it will be much more difficult to get interviews as the competition is getting higher every year. you better get a mid to high 90s score in step 2, have some USCE and USlor. Anyways if you do your best to be a competitive applicant, regardless of your step 1 score, you can make it.
