sarah,28 year old female gives history of hearing voices commenting on her thoughts and actions for last three months. she also says that the newspaper indirectly refers to her. Her symptoms started after she lost his job four months back. He has been working as a consultant in a beauty clinic.her behavior is strange sometimes.she tells that she has become irritable and she has surprised few of her customers with inappropriate sexual comments. she picked up an unnecessary quarell which has surprised all , since she is known her for good behaviour and manners previously. she shows peculiar thinking. There is no history of any substance abuse. Herurine toxicology screen is negative. The most likely diagnosis for this patient is-
a. Schizophrenia
b. Schizophreniform disorder
c. Schizoaffective disorder
d. Brief psychotic disorder
e. Delusional disorder

b. Schizophreniform disorder is the dx at this point bc the duration of her symptoms is less than 6 months but more than a month, if her symptoms persist for more than 6 months then her dx will change to the schizophrenia.

sorry guys,you missed the bus! clinical vignette clearly states that apart from the schizophrenia features , patient portrays MOOD DISORDER .the choice is schizoAFFECTVE(MOOD) disorder. this question appears exclusively in step 2 though.

question undoubtedly tells the mood disorder features, like" she has surprised few customers with inappropriate sexual comments and picked up unnecesary quarell with a neighbour , which tilts the answer infavour of shcizoAFFECTIVE disorder. it is one of those questions on boards which most people gets wrong.

Schizoeffective is the 'waste basket' diagnosis in psychiatry, that is when all other diagnosis dont fit.
anyways thanks for your info, will keep these kinda scenarios in mind
