here is what I know if your target is internal or family medicine, your best shot in my openion is transitional yrear programs. they all have a very strong IM component ( up to seven month in the year) which will give you a very good USCE and possibly strong lors from Internal medicine senior residents and even up to cheif of medicine if you were good enough. one good thing about transitional year that the majority of applicants has no usce in any specialty (otherwise they would follow that specialty). also most of the programs don't require usce for this on the other hand preliminary surgery is good if you apply to surgical specialities but I don't think it would give significant advantage to you if you are applying to any medical specialities, and if I were a program director. I would take the idea that such an applicant is undetermined and don't really love medicine. So if I were you I would go for transitional especially in community hospitals in less glamorous states like wyoming or any thing in the middle and south of the U.S. Note you can apply to transitional year through eras (just pick it from the speciality list). As for me I am applying to preliminary surgery because I want to pursue cat or advanced specialty surgery. I want only surgery so a transitional will not be a good idea for me as for canada they have a independant license exam. so I don't know if they will accept usmle. if they do please tell me I hope this could be useful

Well, yes, transitional is good, but it is VERY hard to get, so I would say in your situation the prelim surgery would be the safer choice

Well don't hesitate to apply as much as you can. even if you pay 3000 or 4000$ , remember you will get it all back if you got even a one year residency

Well...for me, apply wisely. Why will you apply to a school where they never took any IMG/ask for USCE/set limit on graduation year. "As much as you can" is not always the clever way. Contact every single program and ask about their requirements. Apply only to those where you meet their requirements. GL

Well I meant apply to all those who meet your requirement. one thing to point out is that some of the hospitals who require 3 letters of recommendation for us srudents don't count these limitation on imgs. so realize the difference bet img requirement and us student requirment

hi, I have the same question with kemcolian, although I do not apply this year. I am also thinking to use this 1 year to get USCE and recommendation letters. Beefoxy, How hard is it to get the Transitional ? WOuld you like to give some detail? e.g. Compare to catergorical? Graduation year limit? Is a Greencard a must? Will there be any negative effect on Match later on? Thanks

this is a good thread cause even i have somewhat same kinda thinking , why cant i use 1 year USCE next year?cause i m not doing good in this match this year , will it be hopeful next year with this 1 USCE , basically what i came to know is different people have different ideas , some feel surg prelim gives a bad idea that this guy cant get in surg cat or is an unstable guy , but some think it as a USCE ,and some say u r better off without taking surg prelim, this is all for the swtiching the programs from surg to medicine , plz continue this thread
