25 yr old woman w/generalized bone pain for 9yrs
has frequent loose bulky stool, exam shows musle weakness,distend abdom. x-ray shows generlized demineralization of bone.
serum finding of the pt. should be:

Ca.Phosphorus.alkaline phospatase.PTH

a.dec,dec,inc,inc

b.dec,inc,inc,dec

c.dec,inc,inc,inc

d.inc,dec,dec,dec

e.inc,dec,dec,inc

anyone please...

I think the answer is A. Malabsorption leading to hypovitaminosis D.

i think its A too, vit-D def. due to fat malabsorption leading to low Ca++ absorption thus low serum Ca++ which iover the span of nine yrs hv been compensated by inc. in PTH! which wud lower the PO4 level as well!

besides if Ca++ were to increase due to excess bone resoprtion why wud PTH increase then, unless its production gets autonomous; which is not the case here!

ok it looks like A, except why shud there be an incrsd ALP????
in malnutrition or incrsd PTH phosphates decrs and so shud ALP

alkaline phosphatase hydrolyzes phosphate esters which then depending upon the product of conc. of Ca++ and PO4 either gets into solution or precipitated out!

it is indeed a non-specific indicator of bone turn-over and increases with inc. PTH!; because with ensuing bone loss AP wud be activated to reconstitute the bone mass!
