I know there are Sports Medicine, Orthopedics, Acute Care, OB/GYN ( I think this is new), and ER (I think is somewhat new as well) and I have seen Women's Health. There may be more but those are the ones I have heard about.

I do not think there are traditional medicine fellowships like Cardiology, Pulmonology, Hem/Onc although, your focus in FMP is your choice and there are Family Medicine Doctors runnng stress ECG labs in Level 2 and 3 hospitals.

I think it is one year fellowship and you work as a faculty in residency program or med school. And as you know faculty make less than private practice doctors so you can figure it out

With your state MD license you can do any procedure you want.'The problem is that hospitals will not give you priviliges or thatno malpractice insurance will cover you for doing that.
I know some FM that they even do colonocopy and endoscopy in their clinics.
