A newborn has multiple hemorrhagic cutaneous lesions and does not respond to sound. Head CT shows periventricular calcification. Which of the following infectious agents is the most likely cause of this child's presentation?
A CMV
B Herpes simplex virus
C Rubella virus
D Treponema pallidum
E Toxoplasma

Please explain when you answer. Thank you.

The child presents the classic triad of CMV infection:

1. blueberry muffin sd. - cutanate hemorrhages
2. deafness
3. CNS calcifications

so it's A for sure

I agree it could be A , but the butterfly muffin rash appears to be a part of rubella...& I picked this from Manu's links above:

CONGENITAL RUBELLA SYNDROME (CRS). The most commonly described anomalies associated with congenital rubella syndrome are ophthalmologic (cataracts, pigmentary retinopathy, microphthalmos, and congenital glaucoma), cardiac (patent ductus arteriosus, peripheral pulmonary artery stenosis), auditory (sensorineural hearing impairment), and neurologic (behavioral disorders, meningoencephalitis, and mental retardation). Neonatal manifestations of congenital rubella syndrome include growth retardation, interstitial pneumonitis, radiolucent bone disease, hepatosplenomegaly, thrombocytopenia, and dermal erythropoiesis (so called "blueberry muffin" lesions). Mild forms of the disease can be associated with few or no obvious clinical manifestations at birth. The occurrence of congenital defects is up to 85% if infection associated with maternal rash occurs during the first 12 weeks of gestation, 54% during the first 13 to 16 weeks of gestation, and 25% during the end of the second trimester.

In the text there wasn't anything said about no muffins grin. it was klimt who said it.

The thing that didn't fit in the 3 viruses was those skin lesions that I found in the CMV article stated:
Congenital infection has a spectrum of manifestations but usually is silent clinically. Some congenitally infected infants who appear healthy at birth are later found to have hearing loss or learning disability. Approximately 10% of infants with congenital CMV infection have profound involvement evident at birth, with manifestations including intrauterine growth retardation, jaundice, purpura, hepatosplenomegaly, microcephaly, intracerebral calcifications, and retinitis.

What about the CN VIII deafness? I seem to remember it is a presentation of congenital syphilis.

cerebral calcification and deafness can occur with both cmv and toxopalsmosis but im not sure if cutaneous leisons occur with cmv?
