Exacerbation of Myastenia gravis in a patient treated with cholinesterase inhibitors may occur from 2 causes. First,patient may be undertreated, receives suboptimal doses of drug, so not enough ACh is available at neuromusc junction, this is called myastenia crises which is improved by Edrophonium which is a very short acting cholinestersae inhibitor. and second, if patient is overtreated with cholineserase inhibitors, result too high levels of ACH, cholinergic crises, here edrophonium will increase ACH even more causing excesive stimulation and muscle refractory to impulses.,
