Could someone Clear this up? leopard/ young doc/ other forum greats!

An 18 year old female athlete reports easy fatigability and weakness findings on physical examination are unremarkable. Laboratory data are
Serum
Na+ - 141 mEq/L
cl- - 85mEq/L
k+ - 2.1meq/L
HCO3- - 35mEq/L

Urine
Na+ - 80mEq/L
K+ - 170 mEq/L

a) aldosterone deficiency
b) anxiety reaction with hyperventilation
c) diabetes mellitus with ketoacidosis
d) ingestion of anabolic steroids
e) surreptitious use of diuretics

according to dr. Goljan, every athlete on USMLE is on anabolic steroids. Steroids produce, amond their very many side effects, muscle weakness and electrolite imbalance: decreased Ca++, K+ and increase Na+ and glucose.
if anyone has a more complete (or different) explanation, please share it! This particular subject is very board-relevant

well, I ain't such a great, and even if I'm not among those mentioned I'll give a shot:

you can easy see from the lab values that the guy loses K+ and Na+. What? are you stupid? can't you see that the serum Na is within normal limits? Yes, but urine Na is high

Well, let's see... I'd say that's due to a high aldosterone level. The guy absorbes high amount of Na and passes in urine K. Why?

Well, she is taking some king of diuretic. I'd say loop 'cause she looses Cl- too and not Carbonic anhidrase inh 'cause she's got a high HCO3- in serum

also, she's dehidrated which is secondary to diuretic abuse. why would she do such a thing? I don't know! I knew bodybuilders take them to end up showing a higher striation, but athletes? Also, boxers take them to fit into the weight cathegory they want

i agree with the explanation given by manunastai!

it is indeed E

athletes use diuretics to lose wt.!

cmon guys, chk out the scrutiny system for olympics and other events; based on electrolyte testing in urine and serum samples!

anyway, anabolic steroids don't hv mineralocorticoid function!
