A 32 year old male ,from arkansas ,presents in april with fever and headache for the last 2 days.Fever is associated with chills.He also complains of malaise and myalgia.He recalls exposure to tick bite 2 weeks ago while he was walking thru the woods.He denies any neck stiffness or any other neurological abnormality.His vitals are PR=90/min,BP 125/80,temp=102 F,Systemic exam is unremarkable with no rash or any signs of meningeal irritation.Lab testing shows.HCT 29%,WBC count 3000,plt count 80,000.

why is it not lymes disease?

hi,
well this is a classical presentation of ehrlichioses...........
pancytopenia is the hallmark of ehrlichioses along with tick bite........
symptoms too are classical for a case of ehrlichioses........

presence of erythema migrans is highly sugestive of Lyme ds...whereas flu like symp without rash sugest babesioisis or erlichiosis.

complete blood count is normal in lyme.

baabesiosis---> thrombocytopenia and anemia.

erlichiosis--> leukopenia highly sugestive,

the three of them are endemic in similar areas.

yup ehrlichia Chaffeensis causing Human Monocytic ehrlichiosis(HME) Resembles Rocky mounted spotted fever but no rash
