27 year old primarygravida at 39 week gestation , in labour for 10 hours. Over the past 3 hr cervix has remained 90% effaced and 8 cm dilated. Contraction occur every 8 min. Fetal heart rate is 140/min. Her temp is 37 C, B.P 120/80 , pulse 80/min, RR 20/min. Vertex is at left occiput anterior and at 0 station. What is the next appropriate step? a. C-section b. Vaccum delivery c. Oytocin augmentation d. Rotation of head to occiput posterior e. Oral barbiturate administration.

contraction are occuring every 8 mins but should cocur every 2 ins and the foetal herat rate is also normal (no distres)soo action should be taken against the uterine contraction by increasing the oxcytocin drip

when a patient goes beyond three centimeters dilation she is said to be in active labor. This is called failure to progress first oxytocin should be used if this does nort work then proceed to cesarean section

when we have labor arrest in active phase we need to go for C.section.

Its oxytocin augmentation------>she is gettin contractions every 8 mins,contractions shld appear every 1- 2 mins,when the pt has dilated for >8cm,and therefore this lady requires oxytocin,if the q stated that ,inspite of getting good contractions say every 2 - 3mins and still she remained the same,without any dilatation--->this is ARREST in the active phase, and then there is risk for uterine rupture and therefore the answer in that case is defintely C- SECTION...

The only answer is C. section..This is active stage arrest-Go for C.section.

the ans is c section..from 4 cm dilat, its C Section the labour stayed for 10 hrs so most probably the uterus is on its way so suffer atony, so c section( last 3 hrs no progress)

very clear, the anwer is oxytocin augmentation. We go for C-section only when the uterine contraction is normal. However, this case has abnormal uterine contraction
