1.first step in apatient of gout?(red,warm,painful ankle) 2.what to do after confirming diagnosis? 3.diagnosis and treatment if patient returns after six months with red,warm painful knee swelling? 4.same patient has 4 documented episodes of gout despite limiting alcohol and diet...wats d next step? 5.patient is given allopurinol..300mg Dd...he is fine but has another attack after 2 years...wat do u do now?? well people these are d kaplan rheumat qts...if someone has d ans please post them and just do it,u rock

Answers to your questions: 1. Arthrocentesis. 2. NSAID'S (INDOMETHACIN 50 mg 3 times/ day) 3. Again, arthrocentesis--bcoz a different jt is affected and to rule out a deadly, septic jt. Rx: NSAID's & Life style modifications,..low-purine diet, alcohol limitation, avoiding drugs that ppt gouty attacks. 4. Now, pt. fits the criteria for chronic hypouricemic Rx---So, first, know if pt. is a undersecretor/overproducer, by doing 24-Hr urine uric acid levels. 5. Chronic colchicine prophylaxis (0.6 mg /day). Plz, add on any corrections / info.
