Hi, I was reading the diuretics portion from Kaplan LN and FA. The LN notes say that mannitol is used in anuric states but the fA says it is contraindicated in anuria. I think it should be used but is FA wrong or is there some other logic to it? Which one is corrrect? Thanks

Both are correct.Actually use of mannitol in initial ATN (acute tubular necrosis ) is controversial and giving variable results. In the late stages of ARF (acute renal failure) ,it is definitely contraindicated because it expands the ECF -----------> very severe pulmonary edema.Kidneys are not working ,so very difficult to get rid of fluid accumulated in the lungs. Best of luck

KL SAYS MANNITOL IS USED TO PREVENT ANURIA IN RHABDOMYOLYSIS AND HEMOLYSIS. MANNITOL USED IN IMPENDING ACUTE RENAL FAILURE AS IN SHOCK CVS SURGERY,.500 TO 1000 ML OVER 24 HOURS CONTRAINDICATED IF ACUTE RENAL FAILURE HAS ALREADY SET IN.
