which is the most common organism? is it pneumococci or klebsiella, as empirical antibiotic treament would be different for both!

in step 1 pen g was supposed to be given for strep pneumo. but step 2 books rely more on Vanco ( as there is even resistence to ceftriaxone ) for empriic therpay

Risk factors: 1)For penicillin-resistant S. pneumoniae: Previous use (within 3 months)of -lactam antibiotics, alcoholism, age <5 years or >65 years, and (in some areas)residence in a nursing home. hence empiric therapy fo rthese pts is not penicillin.

when resistence is suspected a third generation cephalosproin or FQ ( with enhanced activity against strep eg levofloxacin) is given. if severe, vanco may be given

Well it mean we have to consider every patient having resistant pneumococcus so shuld give CS or FQ to all patient and not use panicillin ?

well in the pts with the risk factors listed we dont give penicillin. otherwise in a low risk out patient with CAP it might be considered.

guys, klebsiella is the most common wrong answer for ' what is the most comon org causing pneumonia in the alcoholic' the answer remains strep for all adults in community acquired pneumonia, be it an alchoholic, elderly, diabetic, hiv,etc

that is what kaplan says!!! thing is klebsiella is most common among alcoholics but the most common org overall in alcoholics remains streps.
