-Whatīs the leading cause of death in pts with RA?? approx 50% of pts die because of this. ??

-Which would be the 1st DMARD (disease modifying antirheumatic drug ) to start in a pt with RA?

-Which are the early radiographic changes in these pts? and which are the joints most commonly affected witht those changes??

-Which are the late radiographic changes?

-Whatīs Felty Sx?

1. Infections , cardiovscular and renal diseses (especially amyloidosis) is the leading cause of death in patients with RA

2. I think it is not known which is the best initial DMARD for patients with RA. Because of safety, convenience, and cost, Hydroxychloroquine can be the initial DMARD of choice for patients with mild disease. because I know MTX is the initial DMARD for patients with severe disease

3. X-Ray- Soft tissue thickening ,Joint space narrowing
joints most commonly affected - PIP’s and MCP
other joints include - WRISTS ,ANKLES ,KNEES ,NECK ,SHOULDERS ,ELBOWS ,HIPS

4. late X-ray changes may be - Subchondral cyst and cortical margin erosions and joint destruction

5. Felty's Sx - RA with Splenomegaly and neutropenia

2. correct, itīs controversial. Others say corticosteroids could work as DMARD, as well (initial).

3. also juxta-articular osteopenia.

remainder is fine.
