An unconscious patient was highly suspected of brainstem lesions. The location of the disease is examed by cold water irrigation to the external auditory meatus. The test shows a left eye slow deviation when irrigation to the left ear. Which of the following is correct?

a. Brainstem intact
b. Bilateral MLF lesion
c. Low brainstem lesion
d. Warm water irrigation is needed for further diagnosis.
e. Nystagmus to the oposite side can be observed.

A is correct and it seems that E is also correct. COWS- Cold water opposite nistagmus, warm water same side nistagmus.

Please give correct answer

The interpretation of the test finding- left vestibular apparatus and the connections are intact- This would involve the upper medulla and pons (for lateral eye deviation). Consciousness is mainly seated in the midbrain reticular formation and bothe hemispheres. So option B and C are wrong.
D- Adding warm water is not really going to add anything much.
E- nystagmus is usually observed in conscious patients. If some one is deeply unconscious, you only see eye deviation (which is just opposite of nystagmus direction-i.e., cold-eye deviation to same side, warm to opp side)
A is relatively right- but could be wrong also as it says "brainstem" is intact. We have no idea whether midbrain is intact. Part of brainstem-pons, medulla is intact would be the most appropriate answer.
