previousely healthy 30 year old woman has a painless lump in her neck for 2 days.her mother have been treated for thyroid tumour at the age of 35 years.her sisiter has raised calcitonin levels.on examination u find a palpable thyroid nodule.serum calcitonin is raised.which of the following shuld be excluded prior to surgical removal of the thyroid mass?

1-lung ca
2-adrenocortical ca
3-parathyroid ca
4-ovarian ca
5-pheocromocytoma

Pheochromocytoma, as they can be lethal because of intractable hypertension or anesthesia-induced hypertensive crises.

the q gives the picture of MEN syndromes. in Men 2 medullary ca of the thyroid is involved. ( suggested by the raised calcitonin). pheochromocytoma is also seen in the condiition. we need to evaluate for it preop coz the stress of surgery may ppt dangerous HTN and hence crisis. look for the pheo beforehand and provide aqequate ALPHA blockade before hand ( remember b blockade is dangerous in the initial phase )

Why not 1-lung ca, it secrets calcitonin too. not any clue for pheochromocytoma in this patient, although it's a comorbidity of MEN2.
