1. Yes, as long as you have your USMLE step 3 scores in your hand and you are ready to start the paperwork. You have to get a license for the state that you are going to be working in and have everything ready for the hospital appointed attorney. 2. Depends if the hospital is cap exempt or non cap exempt. It doesn't matter what it means but the hospital can tell you if they are or not. Cap exempt is best. 3. October? Now you are pushing it. You want an H1 visa, but you don't want to start on time. Residency programs are built to start in July - graduates leave, and they need new residents in the system to take care of the patient population. If it's visa related a hospital might be able to wait some time (we waited 10 weeks for two residents this year), but October? Good luck
