basically it denotes abnormalities of TFTs in pts under the stress of illness. in such situations, TFTs are not reliable indicators of thyroid dz.

In the peripheral tissues T4 adopts two paths
  1. Active path i.e T4 -----------(deiodinase) -------------> T3 (active)
  2. Inactive path i.eT4 ---------------(deiodinase) ----------> rT3 (inactive)

Normally T3>> rT3

In chronic debilitating diseases ,due to some unknown reason ,T4, instead of being converted into T3, is converted more into rT3.So functionally it is a "Hypothyroid like state.I.e rT3>>T3
