45 year old male come for evaluation of 8 month diarrhea. lost 15lbs in 8 month, denies any blood in stool, 24 hr stool reveals fecal fat 10g/day. D-xylose test was performed which shows less than 2g of D-xylose is excreted in 5 hours. D-xylose test was reperformed after antibiotics treatment but excretion still less that 2g in 5hours. what's the diagnosis? A lactose intolerence B bacteria overgroth C pancreatic insufficiency D whipple's disease E terminal ileal disease I feel non of the above answer is correct. But the answer is D-whipple's disease. can anyone tell me why? Whipple's disease not respond to antibiotics?

well whipple's dz will respond to antibiotics but the recovery of villi will take time! none of the other will have reduced absorption of d-xylose
