A young, healthy 25-year-old comes to your office for her yearly female examination. She would like to start taking combination oral contraceptives for birth control. Which of the following are potential side effects?

A. Edema
B. Decreased risk of myocardial infarction
C. Increased risk of endometrial cancer
D. Increased risk of ovarian cancer

C

Estrogen and OCPs is a

Positive risk factor for
  1. Breast cancer
  2. Endrometrial cancer

But Negative risk factor (protective) for

Ovarian Cancer

Best of luck

OCPs prevent pregnancy. so mentrual cyles r continuos. so endometirum is likely to get repetetive prolifration. so enodmetrial ca is likely.

C..

my answer is A

because OC pills does not cause endometrial or ovarian ca. estrogen alone can increase risk of endometrial or ovarian ca but not OCP.

and OCPs does not decrese the risk of MI because it decreases HDL and increase LDL and also increase coagulation.

so the last option is A. edema ( because of water retention)

yes i think claw is right..

  #7

yes its C (ocps r exogenous source of estrogens)

UTERINE CANCER

Carcinoma of the endometrium is the most common female pelvic malignancy. Approximately 40,300 new cases are diagnosed yearly, although in most (75%), tumor
is confined to the uterine corpus at diagnosis and therefore most can be cured. The 7,000 deaths yearly make uterine cancer only the eighth leading cause of cancer
death in females. It is primarily a disease of postmenopausal women, although 25% of cases occur in women Eastern Europe and the United States and uncommon in Asia.
Phenotypic characteristics and risk factors common in patients with endometrial cancer include obesity, altered menstruation, low fertility index, late menopause,
anovulation, and postmenopausal bleeding. Exposure to unopposed estrogen from either endogenous or exogenous sources may play a central etiologic role.
Women taking tamoxifen for breast cancer treatment or prevention have a twofold increased risk.
Endometrial carcinoma occurs most often in the sixth and seventh decades of life. Symptoms often include abnormal vaginal discharge (90%); abnormal bleeding
(80%), which is usually postmenopausal; and leukorrhea (10%). Evaluation of such patients should include a history and physical and pelvic examinations followed by
an endometrial biopsy or a fractional dilation and curettage. Outpatient procedures such as endometrial biopsy or aspiration curettage can be used but are definitive
only when positive.

I think it is A.

C and D are excluded

A. Edema (major side effect associated with progesteron in COCs)

B. Decreased risk of myocardial infarction (I think this should be Increased not decreased as decreased risk of myocardial infarction is not a side effect smiling face, or will be excluded as a correct answer, this is an uncommon side effect but its risk increase if the woman is a smoker or overweight, there is an increased risk of thromboembolism with COCs)

C. Increased risk of endometrial cancer (COCs actually decrease the risk of developing endometrial cancer I think due to its progesteron content in contrary to your beliefs) so excluded

D. Increased risk of ovarian cancer (COCs decrease the risk of developing ovarian cancer)

feel free to correct me if I'm wrong wink

answer is A because of her young age edema rather than uterine ca is appropriate
