54 year old obese man presents for a routine physical examination.He was diagnosed with type 2 diabetes 1 year earlier.He has been moderately compliant with dietry precautions and his morning glucose has been persistently between 150 and 200 mg/dl.He is therefpre started on glipizide.One month later metformin is added bcz of continues poor control.His other medications are propranolol and nifidepine for HTN and naproxen which he began aprrox 2 weeks ago for severe knee pain due to osteo arthritis.On physical exam his BP is 154/92,and he has soft 4th heart sound.The remainder of physical exam is normal.His routine electrolytes are checked and revealed a BUN of 29 mg/dl and a creatinine of 1.8 mg/dl.both had been normal one year earlier.Which of the following medications is likly responsible for the increase in the BUN and creatinine?

A.Glipizide

B.metformin

C.naproxen

D.nifedipine

E.propranolol

yes correct.

metformin=lactic acidosis

glipizide=hypoglycemia

nifedipine and propranolol are not associated with nephropathy

NSAIDS cause interstitial nephritis...
