when it is mentioned that a person has focal neurological exam,what exactly does it mean?

symptoms pointing towards a focal lesion eg rt hemiplegia or UMN signs in the lower limb, or a cranial nerve deficit
vs those affecting generalized eg increased ICP--bilateral papilledema, vomiing, early morning headaches
