A teenage patient is brought to the ER,he is agitated,belives he can fly
her frind tells the staff that he consumed some extarct made from weeds,patient has elevated body temparature,pupils are dilatated and tachicardia
what is the best treatement ?

a.-Carbachol
b,-ipratropium
c.-methscopolamine
d.-nesotigmine
e.-physostigmine

Atropine blocks the muscuranic receptors . Physostigmine will reduce the symptoms by competitive stimulation.

Actually S/S show that he is poisoning with some parasympatholytic agents like atropine.It has effects on
  1. CNS = agiation ,hallucinations
  2. Peripharal = dilation of pupil + tachycardia + fever due to lack of sweating

There are two drugs in the scenario which can combat it effectively
  1. Neostigmnin = acts only on Peripherally because does not cross BBB
  2. Physsotigmnin = Crosses BBB ,so effective
    1. centrally
    2. peripherally
